1.1.1 Incident Category

1.1.2 Site Description
The Mid-Valley Pipeline carrying crude oil from Hebron, Kentucky to Lima, Ohio released product into an un-named stream leading into a roadside pond along East Miami River Road and adjacent to the Great Miami River. The crude oil was contained within the pond. No observable product or sheen has been identified on the Great Miami River, which is located approximately 1,500 feet west of the pond. The pipeline is operated by Sunoco Pipeline LP and was shut down at 2232 hours on March 17th (restarted on March 23). The 20-inch pipeline was installed in 1950. It was estimated that approximately 240 barrels (approximately 10,000 gallons) were released to the stream and pond. Approximately 1 mile of stream was impacted with crude oil. Sunoco revised the spill amount to potentially 500 barrels (21,000 gallons) on March 21, 2014.
1.1.2.1 Location
The majority of the Site is located within the Oak Glen Nature Preserve on East Miami River Road in Cincinnati, Ohio. The stream runs down a steep hillside into a pond located across the street from a stone quarry located at 8953 East Miami River Road. A command post has been established on East Miami River Road (Dravo Park) north of the pond. Sunoco Logistics/Mid-Valley has obtained signed access agreements with Great Parks of Hamilton County and numerous private land owners. Oak Glen Nature Preserve representatives (Great Parks of Hamilton County) were onsite 3/18/14 and are part of Unified Command.
1.1.2.2 Description of Threat
There is a confirmed release of approximately 21,000 gallons of crude oil into a stream which discharges into a pond within the Oak Glen Nature Preserve. Due to the quantity of crude oil released and proximity to the Great Miami River, there is a threat of a release to the Great Miami River.
1.1.3 Preliminary Removal Assessment/Removal Site Inspection Results
On March 18, EPA OSC Renninger, Ohio EPA OSC Dale Farmer, Ohio EPA OSC Bill Lohner, EPA's START contractor, and Sunoco Logistics/Mid-Valley walked about a mile from the pond and along the stream and ending at the pipeline discharge point, which is located where the Mid-Valley pipeline crosses the stream.
Visible crude oil was observed on the surface of the pond and along the approximately 1-mile stretch of stream bed. The terrain around the impacted stream is heavily wooded and steep. Heavy pockets of crude oil was observed at numerous locations within the stream.

2. Current Activities
|
|
|
2.1 Operations Section
|
|
|
|
2.1.1 Narrative
A Unified Command has been established with EPA, Ohio EPA, Great Parks of Hamilton County, Colerain Township and the responsible party (RP) Sunoco Logistics/Mid-Valley. The RP has contracted with SWS Environmental Services, Clean Harbors, Environmental Restoration, Dillon, and McCutcheon to conduct product recovery and clean-up operations. OTIS and DOT were on site to assess the damaged pipeline. In addition, Hull & Associates (Hull) has been contracted as its environmental consultant. Hull is conducting air monitoring and is leading environmental assessment efforts. A command post has been established in the parking lot of Dravo Park, north of the pond. Coordinating agencies can be found in Section 3.
The following 4 working zones have been established:
- Zone 1 - This is the area where the pipeline breach occurred.
- Zone 2 - This the 1-mile stream section impacted by crude oil from the pipeline break (Zone 1) to the influent of the underflow dame. Zone 2 is divided into 7 segments (Zones 2A-01, 2A-02, 2B-01, 2B-02, 2C-01, 2C-02 and 2C-03.
- Zone 3 - Begins at the outflow of the underflow dam and includes the pond where the crude oil was initially discharged into.
- Zone 4 - This is the area west of the pond (Zone 3) towards the Great Miami River.

March 25 and 26, 2014
IMT conducted the Planning Cycle for a 24-hour operational period and generated an IAP for each day.
Zone 1
The pipeline has been repaired and the line has been restarted per DOT.
Oil-contaminated soil was excavated from around the pipeline and placed into rolloff boxes. A post excavation soil sampling plan was developed and implemented. Post excavation soil sampling data showed BTEX concentrations less than site action levels.
An access road was constructed from Tree Top Lane to the top of the hill which leads down to the pipeline that was temporarily repaired.
Zone 2
Full-scale flushing along the stream in Zone 2 continued (300
gal/min) using
pond water pumped overland to Zone 1. SCAT personnel reporting that the
water flushing was performing as planned. Pockets of oil that are in
the stream are being flushed and pushed where it can eventually be
collected using skimmers and vacuum trucks at the underflow dam.
SCAT team recommendations being implemented by Operations on oily debris removal in the stream to increase
flow of the oil. SCAT recommendations incorporated into the IAP.
Zone 3
Manual removal of residual oil and sheen continued at the pond. Oil-contaminated debris continued to be removed from the pond. All skimming operations completed.
Thermal treatment was initiated on the oily residue ring around the shoreline of the pond. The Colerain Fire Department provided oversight of all thermal treatment operations.
Hull collected surface water samples for laboratory analysis from
two locations on the pond on a daily basis. Hull also collected a
water sample from the discharge water from the underflow dam outlet.
Air sparging operations continued, as three aerators were placed and operating in the pond.
Zone 4
Daily inspections continue with no oil or sheen observed.
Other Information
EPA START contractor assisting with
assessment, documentation, sampling and monitoring activities.
No
wastes have been transported off-site. UC approved the RP waste disposal plan. RP will begin scheduling waste to be transported for off-site disposal.
An air monitoring plan continued to be implemented by Hull. Hull is monitoring 15 air sampling locations. No issues with elevated VOCs are being observed.
The
Wildlife/Sensitive Species Plan for managing wildlife encountered continued to
evolve with input from experts. Cardno ENTRIX implementing the Salamander Habit and Survey Action Plan. Tri-State Bird & Rescue on site to assess and respond to animals discovered which are impacted. Five additional salamander traps were installed.
RP investigating long-term office space and drafting a transition plan. RP began the development of an equipment decontamination plan.
RP estimated 168 people on site.
March 27, 2014
IMT conducted the Planning Cycle for a 24-hour operational period and generated an IAP.
Zone 1
Orange fencing was placed around the "temporarily" repaired pipeline for added security.
Zone 2
SCAT recommendation to suspend full-scale flushing along the stream in Zone 2. SCAT assessing streamline Zone 2A-01. Operations focusing cleanup efforts in Zone 2A-01. Per the Zone 2 Exit Plan, SCAT and Ohio EPA will be conducting the inspection of the stream once Operations feels a segment of stream meets Zone 2 criteria and, if necessary, approval for release.
SCAT team recommendations being implemented in the stream to increase
flow of the oil. SCAT recommendations incorporated into the IAP.
Zone 3
Manual removal of residual oil and sheen continued at the pond.
Hull collected surface water samples for laboratory analysis from
two locations on the pond on a daily basis. Hull also collected a
water sample from the discharge water from the underflow dam outlet.
Air sparging operations continued, as three aerators were placed and operating into the pond.
Zone 4
Daily inspections continue with no oil or sheen observed.
RECOVERY UPDATE
As of 1500 hours on 3/27/14, total
liquid recovery was 38,044 gallons (oil and water), of which 17,487 gallons was oil.
Other Information
EPA START contractor has mobilized to the site to assist with
assessment, documentation, sampling and monitoring activities.
Hull using rotosonic drilling to install groundwater monitoring wells around the site. Four of 9 wells have been installed.
Environmental stated that 17 rolloff boxes are awaiting off-site disposal. One rolloff box containing oil-contaminated sorbents and PPE was shipped for off-site disposal to the Franklin County Sanitary Landfill, located in Grove City, Ohio.
An air monitoring plan continued to be implemented. 14 air sampling
locations being monitored. No issues with elevated VOCs being observed.
The
Wildlife/Sensitive Species Plan for managing wildlife encountered
continued to
evolve with input from experts. Cardno ENTRIX implementing the
Salamander Habit and Survey Action Plan. Tri-State Bird & Rescue on
site to assess and respond to any animals discovered which are
impacted. Thirteen crawfish were released into Shaker Trace creek.
Logistics signed a lease for long-term office space at a facility approximately 5 miles from the current command post. A transition plan will be developed. The current command post tents will be cleaned out by Friday April 4th and will be demobilized on Saturday April 5th.
Safety developed procedures in the event lightning or heavy winds are encountered. Safety developed a Traffic Control Plan.
Ohio EPA provided a salamander expert to oversee the recovery and release in the environment any captured salamanders.
This is the last day for scheduled media briefings.
RP estimated 148 people are on site.
March 28, 2014 (as of 1100 hours)
IMT conducted the Planning Cycle for a 72-hour operational period and generated an IAP.
Zone 1
Operations to install a silt fence around the pipeline.
Zone 2
Operations aggressively using its resources to remove oil-contaminated leaves, brush and other vegetation from the stream. SCAT inspected reaches of the stream, as part of the Zone 2 Exit Plan. Additional SCAT recommendations are being implemented including contaminated debris removal and exposing contaminated sediment for additional flushing operations.
A modified flushing operation will be conducted over the next few days. A combination of high flow and low flow flushing will be determined by SCAT and Operations.
Thermal treatment operations to be conducted, as directed by SCAT. Thermal treatment operations to potentially be focused on oil-stained rocks.
Zone 3
Manual removal of residual oil and sheen continued at the pond. Pockets of oil being removed using absorbent pads.
Hull collected surface water samples for laboratory analysis from
two locations on the pond on a daily basis. Hull also collected a
water sample from the discharge water from the underflow dam outlet.
Air sparging operations continued, as three aerators were placed and operating into the pond.
Zone 4
Daily inspections continue with no oil or sheen observed.
RECOVERY UPDATE
As of 0300 hours on 3/28/14, total
liquid recovery was 39,971 gallons (oil and water), of which 17,737 gallons was oil.
Other Information
EPA START contractor has mobilized to the site to assist with
assessment, documentation, sampling and monitoring activities.
Hull using rotosonic drilling to install groundwater monitoring wells around the site. Four of 9 wells have been installed. All wells scheduled to be installed by Saturday, March 29th.
Environmental Unit updated Unified Command that waste is being shipped off site today and the goal is to remove most if not all of the waste by the end of the weekend. Rolloff boxes containing oil-contaminated sorbents and PPE and rolloff boxes containing oil-contaminated soil are awaiting off-site disposal to the Franklin County Sanitary
Landfill, located in Grove City, Ohio. Recovered oil will be pumped into tanker trucks and shipped to Sunoco's Hebron, KY facility for re-use.
An air monitoring plan continued to be implemented. 11 air sampling
locations being monitored. No issues with elevated VOCs being observed.
The

2.1.3 Enforcement Activities, Identity of Potentially Responsible Parties (PRPs)
Sunoco Pipeline LP and Mid-Valley Pipeline are the responsible parties. A Notice of Federal Interest was issued by OSC Renninger to Sunoco Pipleine LP representatives at 0800hrs on 3/18/2014.

Security
The
security badging process has been fully implemented. All personnel are
required to have a badge and as they enter the site, the badges are
scanned.
