1.1.1 Incident Category
Emergency Response to a cyanide spill in Shantee Creek. The source of the spill is unknown.

1.1.2 Site Description
The Site is situated in northeast Toledo, Lucas County, Ohio and involves Shantee Creek. At the time of the initial response, the visually impacted portion of the creek spanned from approximately the intersection of West Laskey Road and Lewis Avenue, to a point east of North Detroit Avenue. Shantee Creek is a shallow drainage creek, approximately 15 feet in width, and 2 feet in depth.  The North Maumee Bay of Lake Erie is located approximately 6.25 river miles east of the impacted portion of Shantee Creek.

1.1.2.2 Description of Threat
On 8/15/2011, a spill was reported in Shantee Creek to the City of Toledo by a resident/business owner who reported a orange discoloration of the water and dead fish within the creek.

1.1.3 Preliminary Removal Assessment/Removal Site Inspection Results
The City of Toledo collected a water sample 500 feet east of the intersection of Laskey Road/Lewis Avenue that showed 6.5mg/L cyanide. Another sample collected at North Detroit Avenue contained 1.5mg/L cyanide. The City of Toledo also found elevated levels of zinc in the water samples.

2.1.2 Response Actions to Date
On August 17, 2011, at the request of Ohio EPA, U.S.EPA along with START and ERRS mobilized to the Site. On arrival to the Site, START began air monitoring and water quality measurements at five monitoring locations placed along Shantee Creek, spanning from North Detroit Avenue to just east of the West Laskey Road/Lewis Avenue intersection. START monitored the ambient air for levels of CO, VOC, LEL, H2S, O2, HCN, and NH3 and monitored water quality for pH, temperature, conductivity, and DO. The ERRS deployed air sparging within Shantee Creek (just east of N.Detroit Ave) to aid in the removal of cyanide in water.

On August 18, 2011, START collected 10 water samples along Shantee Creek, spanning from just east of the West Laskey Road/Lewis Avenue intersection to the Raintree Village trailer park at Raintree Parkway and East Alexis Road, which showed a drop in cyanide to less than 1 mg/L. In addition, START and ERRS walked Shantee Creek from North Detroit Avenue to the intersection of West Laskey Road/Lewis Avenue collecting water quality measurements and noting any discolored water, fish kills, or observations of importance - no source was identified.

Ohio DNR, which had documented fish kills earlier in the week, found no new fish kills, and reported healthy fish had returned to the previously impacted areas.
