1.1.2.1 Location

1.1.2.2 Description of Threat
OSC Fife was informed of a refinery fire via the media. OSC Fife contacted the RP, but was given very little information on the situation and any impacts associated with the incident. The RP informed OSC Fife that a fire had occurred at their facility causing a release of diesel fuel, but the fire and firewater had been contained. Several residential areas, and the Arkansas River are located adjacent to the facility. OSC Fife dispatched OSC Brescia to the site to gather more information and to determine if any off-site impacts occurred.

1.1.3 Preliminary Removal Assessment/Removal Site Inspection Results
OSC Brescia and two EPA START personnel arrived on site at 1345 and met with the RP's environmental manager. The manager informed OSC Brescia that the fire had began at 0200, and was extinguished at 0415 by on-site fire personnel. The fire originated from the refineries distillate hydrotreater which had a faulty flange causing a pressurized release of diesel fuel. The diesel fuel ignited and burned, causing a fire and release of fuel/firewater onto the property. The RP manager informed OSC Brescia that a large quantity of diesel fuel burned during the incident and that approximately 200 barrels were released into their waste water treatment system on site and was contained on site. The RP manager informed OSC Brescia that the refineries safety team conducted air monitoring utilizing five gas meters around the perimeter of the fire during the incident. OSC Brescia requested a copy of the air monitoring data and the manager informed OSC Brescia that he would provide it on 08/03/2012. The RP manager informed OSC Brescia that no significant peaks occurred during their air monitoring activities which included monitoring for VOCs, H2S, and CO. OSC Brescia requested that the RP give them access to the waste water treatment ponds to view any collected firewater and to the incident location to view the damage. The RP provided a visit to the ponds and to the distillate hydrotreater. No offsite release of diesel fuel was observed by the OSC. OSC Brescia requested final calculations from the RP once they were known and for the air monitoring data for review.

OSC Brescia asked the RP manager why the facility did not report the incident to the NRC, and the RP Manager informed OSC Brescia that no diesel fuel migrated off-site or into any waterway, and that no known constituents exceeded any reportable quantities for an air release.

OSC Brescia and EPA START demobilized from the site.
