1.1.2 Site Description
1.1.2.1 Location
The location of the leaking well is situated along Tucker Hollow Road, approximately 2.2 miles from it's intersection with Heflin Road, in Hartford, Ohio County, KY. The GPS coordinates for the well are N37.52742932, W87.0293453. The well is situated on Parcel ID 42-27B1 which is currently owned by Patricia and Rhonda Humphreys since June 27, 2006. The parcel totals 15.1820 acres.
1.1.2.2 Description of Threat
Crude oil bubbling up at land surface and discharging to an unnamed tributary to Barnett Creek, which flows to the Rough River and subsequently to the Green River, a navigable water of the United States.
1.1.3 Preliminary Removal Assessment/Removal Site Inspection Results
On 4/15/2014, the the OSC visited the site with the Kentucky Department for Environmental Protection Environmental Response Team (KDEP ERT) and the Kentucky Department of Natural Resources Division of Oil and Gas (KDOG). All parties observed crude oil bubbling up at land surface from what appears to be an abandoned oil production well. It is suspected that the well is uncased, as the oil is migrating along shallow bedrock and is seeping into an unnamed tributary to Barnett Creek located about 15 feet away from the suspected source.

2.1.2 Response Actions to Date
On 4/15/2014, the OSC and the KDEP ERT member deployed sorbent boom and pads to recover a small quantity of oil that was observed in the creek. Also on this date, the OSC issued a verbal Pollution Response Funding Authorization (PRFA) to KDEP to stabilize the oil discharge at the source and to remove oil from the creek.
On 05/12/2014, CMC and OTIE mobilized to the site to initiate well abandonment activities. From 5/12/2014 through 5/15/2014, site work consisted of preparing the site for well plugging activities. Barnett & Smith Well Services were retained by CMC to wash and clean the borehole, and to plug the well with cement. On 5/16/2014, well plugging operations began. The progress was hindered by an obstruction downhole and on 5/17/14, work resumed with the use of a power swivel to advance the hole further.
On 5/17/14, operations were successful in getting down to 350 feet below land surface. Work was suspended at the end of the workshift on Saturday for the weekend.
2.1.3 Enforcement Activities, Identity of Potentially Responsible Parties (PRPs)
Under the Oil Pollution Act, and specifically as it pertains to abandoned onshore facilities, Responsible Parties (RPs) are the owner and/or operator (o/o) of the facility at the time of abandonment. There are no records in the KDOG database for the leaking well. At the present time, there are no RPs identified for this incident. EPA will inquire of the landowner and past landowner as to whether any oil production activity took place on their parcel during the time of ownership. EPA will also research the existence of any past oil and mineral leases to further investigate the possibility of identifying former o/o of the facility.

2.2.1 Anticipated Activities
2.2.1.1 Planned Response Activities
On 5/19/2014, work is scheduled to resume and well plugging will begin as soon as it is feasible. The OSC requested technical assistance from the US EPA Environmental Response Team (ERT) for advising on well abandonment procedures. The ERT rep is onsite and will remain until the hole can be advanced to the bottom of the borehole. Following completion of well abandonment activities, the OSC will coordinate with landowners and have the ERRS contractor resore the site to original conditions.
2.2.1.2 Next Steps
The OSC will meet with with KDEP representatives and coordinate closeout procedures for the underflow dam in the small tributary adjacent to the well. The well plugging process may force out some crude oil pooled in the subsurface and there may be a small volume residual oil removal requirement for a few months after the well is plugged. Issue the TO to prepare the site, plug the well and to remove the oil discharge from surface water.
