1.1.1 Incident Category
This Incident was responded to as a potential pollutant/contaminant release.

1.1.2 Site Description

US EPA Region 4 received a request for assistance from NC DENR DWQ as well as a NRC Report (NRC#917748) on September 14th regarding a release of unknown sludge to Cabin Creek, which is a tributary to Beaver Dam Creek which eventually discharges to the Cape Fear River. The release was reported to NC DENR DWQ on September 10th as a anonymous call. NC DENR investigated the complaint and confirmed the release, as they observed the sludge, collected samples and performed some water quality monitoring. The sample results indicated elevated levels of fecal e-coli and very low dissolved oxygen levels.
EPA conducted a emergency response and met NCDENR on September 15th. US EPA observed waste water treatment sludge in Cabin Creek at the House of Raeford chicken processing plant. Upstream from the chicken plant had no evidence of sludge. DENR performed D.O. monitoring upstream from the plant, D.O. was above 4 which would be normal. Downstream of the House of Raeford Plant, the D.O. was less than 1, approximately 0.13. US EPA believes that the source of the sludge release is the House of Raeford, based on field observations, location of the sludge and data collected by NC DENR.

1.1.2.2 Description of Threat
A large quantity of waste water treatment sludge released to a water way.
Elevated levels of fecal e-coli, low dissolved oxygen thus a potential for a "fish kill" downstream.

1.1.3 Preliminary Removal Assessment/Removal Site Inspection Results
Waste Water Treatment Sludgte was visible in Cabin Creek next to and downstream of the House of Raeford chicken processing plant.  Cabin Creek Dissolved Oxygen levels were normal (4.0) upstream from the chicken processing plant. Cabin Creek Dissolved Oxygen levels were low (0.13) next to and downstream of the chicken  processing plant.

2. Current Activities
  2.1 Operations Section
    2.1.1 Narrative
EPA met with the NC DENR, House of Raeford Operations Manager, Plant Manager and Treatment Plant Operator to develop a plan forward.
2.1.2 Response Actions to Date
House of Raeford will submit a written plan and submit it to NC DENR for review, agreement was reached that House of Raeford would continue to collect sludge from Cabin Creek and Beaver Dam Creek using vac trucks, containment boom would be placed downstream at Sheffield Bridge, vac trucks would be placed at Sheffield Bridge for collection of sludge. Aeration operations will also begin at Sheffield Bridge.

US EPA believes that the source of the sludge is the House of Raeford based on field observations and water quality findings.

House of Raeford has agreed to continue sludge collection operations behind the plant, a containment boom will be placed at the Sheffield Rd Bridge, vac trucks will be mobilized to the Sheffield Rd Bridge location for collection operations and a aerator will be mobilized to the Sheffield Rd Bridge location for aeration.
