On March 15, 2004 at 2315 hours, RIDEM personnel requested that OSC Jakabházy respond to a release of sodium permanganate at the Phillips Components facility in North Smithfield, RI. The facility was in the process of being sold, and is currently listed as a Rhode Island Brownfields site. The sodium permanganate had been injected through a series of wells on March 14, 2004 to enhance the remediation of chlorinated solvents. The wells had intersected a previously unknown pipe which led to the storm drain system which drained to the Branch River.

RIDEM and EPA oversaw the site cleanup activities until the PRP hired Shaw Environmental to complete the restoration of the river and the storm drain system.
