On May 22, 2011, US EPA received NRC Report # 977021. The NRC Report noted: CALLER IS REPORTING AN UNKNOWN SHEEN SIGHTING. EXACT SOURCE OF THE SHEEN IS UNKNOWN AT THIS TIME. CALLER SUSPECTS THE MATERIAL IS COMING A SEWER OPENING AROUND A MALL AREA NEARBY THE LOCATION.

On May 21, 2011, US EPA tasked START to investigate the spill. OEPA OSC Dale Farmer and START were on-scene on May 21, 2011 and observed a white substance in an unnamed creek adjacent to a residential area. The source of the spill was an outfall that was tracked back to a Staples Store that had previously discharged a combination of floor wax remover and floor wax (pH as high as 11).

On May 21, 2011, OEPA mobilized EEI to remove white liquid from the unnamed creek utilizing a vacuum truck.
