1.1.1 Incident Category
Inactive and abandoned natural gas production well.
1.1.2 Site Description
1.1.2.1 Location
The site is located at 11767 Kentucky Route 194, Prestonsburg, Floyd County, KY. The coordinates for the well are N 37.6880667 and W82.5947333. The well is immediately adjacent to an unnamed surface water body which is a tributary to Johns Creek which is located about 1,200 feet downgradient of the well site.
1.1.2.2 Description of Threat
Crude oil discharged from the gas well into Johns Creek, which forms Dewey Lake, and which discharges into the Levisa Fork of the Big Sandy River approximately 23.5 miles downstream of the leak site. The Big Sandy River is a navigable water of the United States.
1.1.3 Preliminary Removal Assessment/Removal Site Inspection Results
On 11/19/2013, the Kentucky Department of Environmental Protection (KDEP) responded to a report of an oil spill into Johns Creek. The KDEP responder observed crude oil in the small unnamed tributary of Johns Creek. The oil was observed to be coming out around the casing of an old gas well located on the right hand side of the creek approximately fifty feet from the last residence in the hollow. A responsible party could not be located for the well, so a contractor was hired to perform the clean-up. EPA OSC Ball issued a Pollution Response Funding Authorization (PRFA) to KDEP to cover the costs of the oil spill response.
On 12/09/2013, OSC Smith and KDEP visited the site to inspect the well site. At that time, no crude oil was observed at land surface, although a rainbow sheen was present in the small tributary. (See pictures under the images tab of the website www.epaosc.org/deweylakeoilwell).
On 09/12/2014, the Kentucky Division of Oil and Gas (KDOG) referred this well to EPA for plugging and proper abandonment. KDOG has classified the well as Permit No. N22058, and labeled it Debbie Jarrell Well #1.
On 10/3/2014, the OSC contacted Nytis Exploration (Nytis), who holds a lease to operate gas production wells on this parcel. The OSC inquired about production records for the November 2013 timeframe, specifically in an attempt to see if any decrease in production at that time could account for buildup of pressure at Jarrell #1. On 10/9/2014, Nytis responded to the OSC's request. In their response, Nytis disclosed that there had been a decrease in gas production at the saem time that the oil discharge had been discovered. The OSC then contacted the property owner, who confirmed that the oil discharge had subsided in the period of time when Nytis subsequently returned to full gas production on this lease.
On 11/05/2014, the site was re-named from Dewey Lake Oil Well to Debbie Jarrell Well #1.

KDEP deployed boom into John's Creek to contain the spill. Cleanup commenced on November 20, 2013 and included construction of a containment pit around the well and an underflow dam in the unnamed tributary. The cleanup activities were completed the following day, and the remaining activities consisted of periodic checks to ensure the absence of crude oil in surface water
2.1.3 Enforcement Activities, Identity of Potentially Responsible Parties (PRPs)
There are no records for this well in the Kentucky Geological Survey Oil and Gas Database. On 1/17/2014, the KDOG sent a notice of violation to the unknown operator, indication that the well was improperly abandoned. The OSC will attempt to identify PRPs for this facility, which will include searching records to investigate the history of oil and gas development on this parcel.
