1.1.1 Incident Category

1.1.2 Site Description
The Mid-Valley Pipeline carrying crude oil from Hebron, Kentucky to Lima, Ohio released product into an un-named stream leading into a roadside pond along East Miami River Road and adjacent to the Great Miami River. The crude oil was contained within the pond. No observable product or sheen has been identified on the Great Miami River, which is located approximately 1,500 feet west of the pond. The pipeline is operated by Sunoco Pipeline LP and was shut down at 2232 hours on March 17th (restarted on March 23). The 20-inch pipeline was installed in 1950. It was initially estimated that approximately 240 barrels (approximately 10,000 gallons) were released to the stream and pond. Approximately 1 mile of stream was impacted with crude oil. Sunoco revised the spill amount to 500 barrels (21,000 gallons) on March 21, 2014.
1.1.2.1 Location
The Site is located within the Oak Glen Nature Preserve and two private parcel lots on East Miami River Road in Colerain, Ohio. The stream runs down a steep hillside into a pond located across the street from a stone quarry located at 8953 East Miami River Road. A command post was established on East Miami River Road (at Dravo Park) north of the pond. Sunoco Logistics/Mid-Valley has obtained signed access agreements with Great Parks of Hamilton County and numerous private land owners. Oak Glen Nature Preserve representatives (Great Parks of Hamilton County) and Colerain Township representatives were onsite 3/18/14 and are part of Unified Command.
1.1.2.2 Description of Threat
There was a confirmed release of approximately 21,000 gallons of crude oil into a stream which discharges into a pond within the Oak Glen Nature Preserve. Due to the quantity of crude oil released and proximity to the Great Miami River, there is a threat of a release to the Great Miami River.
1.1.3 Preliminary Removal Assessment/Removal Site Inspection Results
On March 18, EPA OSC Renninger, Ohio EPA OSC Dale Farmer, Ohio EPA OSC Bill Lohner, EPA's START contractor, and Sunoco Logistics/Mid-Valley walked about a mile from the pond and along the stream and ending at the pipeline discharge point, which is located where the Mid-Valley pipeline crosses the stream.
Visible crude oil was observed on the surface of the pond and along the approximately 1-mile stretch of stream bed. The terrain around the impacted stream is heavily wooded and steep. Heavy pockets of crude oil was observed at numerous locations within the stream.

2. Current Activities

|
2.1 Operations Section
|
|
|
|
2.1.1 Narrative
A Unified Command was established with EPA, Ohio EPA, Great Parks of Hamilton County, Colerain Township and the responsible party (RP) Sunoco Logistics/Mid-Valley. The RP has contracted with SWS Environmental Services, Clean Harbors, Environmental Restoration, Clean Venture, Dillon, and McCutcheon to conduct product recovery and clean-up operations. OTIS and DOT were on site to assess and temporary repair the damaged pipeline. In addition, Hull & Associates (Hull) has been contracted as its environmental consultant. Hull is conducting air monitoring and is leading the environmental assessment efforts. A command post was established in the parking lot of Dravo Park, north of the pond. Coordinating agencies can be found in Section 3.
The following 4 working zones were established:
- Zone 1 - This is the area where the pipeline breach occurred.
- Zone 2 - This the 1-mile stream section impacted by crude oil from the pipeline break (Zone 1) to the influent of the underflow dame. Zone 2 is divided into 9 segments (Zones 2A-01, 2A-02, 2B-01L, 2B-01R, 2B-01-SEEPAGE, 2B-02, 2C-01, 2C-02 and 2C-03.
- Zone 3 - Begins at the outflow of the underflow dam and includes the pond where the crude oil was initially discharged into.
- Zone 4 - This is the area west of the pond (Zone 3) towards the Great Miami River.

Zone 1
No operations conducted in Zone 1. Pipeline reopened on March 23, 2014.
Zone 2
1.3 inches of rainfall fell, causing the stream velocity to be approx 500 gallons per minute. The high velocity in the stream pushed oil and oil-contaminated leaves and debris to the underflow dam, where it was eventually collected by Operations.
Zone 3
Operations contained and removed oil-contaminated debris within the pond.
Hull collected surface water samples for laboratory analysis from
two locations on the pond on a daily basis. Hull also collected a
water sample from the discharge water from the underflow dam outlet.
Air sparging operations continued, as three aerators were placed and operating into the pond.
Zone 4
Daily inspections continue with no oil or sheen observed.
Other Information
EPA START contractor was on site to assist with
assessment, documentation, sampling and monitoring activities.
Hull suspended rotosonic drilling operations due to safety concerns from the weather. Drilling operations to resume tomorrow.
The Environmental Unit reported that tankers of liquid waste and rolloff boxes of oil-contaminated solid waste were shipped for off-site disposal. Rolloff boxes containing oil-contaminated
sorbents and PPE and rolloff boxes containing oil-contaminated soil are being shipped for off-site disposal to the Franklin County Sanitary
Landfill, located in Grove City, Ohio. Recovered oil is being pumped
into tanker trucks and shipped to Sunoco's Hebron, KY facility.
An air monitoring plan continued to be implemented. 11 air sampling
locations being monitored. No issues with elevated VOCs being observed.
The
Wildlife/Sensitive Species Plan for managing wildlife encountered
continued to be implemented. Cardno ENTRIX implementing the
Salamander Habit and Survey Action Plan. Tri-State Bird & Rescue on
site to assess and respond to any animals discovered which are
impacted.
March 30, 2014

around the site. 9 of 9 wells have been installed with OEPA oversight.
An air monitoring plan continued to be implemented. 11 air sampling
locations being monitored. No issues with elevated VOCs being observed.
The
Wildlife/Sensitive Species Plan for managing wildlife encountered
continued to be implemented. Cardno ENTRIX implementing the
Salamander Habit and Survey Action Plan. Tri-State Bird & Rescue on
site to assess and respond to any animals discovered which are
impacted. 55 total wildlife collected and assessed.
Logistics reported that about 40 pieces of equipment and some labor force were demobilized. Equipment is being decontaminated at the decontamination pad prior to demobilization.
March 31, 2014
Emergency operations being transitioned into Remediation operations. Ohio EPA to oversee remediation operations. Once remediation operations are completed, restoration operations will be conducted. Great Parks of Hamilton County will oversee restoration activities.
EPA demobilized mobile command post and all personnel following 1600 hours UC briefing.
Zone 1
No work was conducted in Zone 1. Permanent repair of the pipeline to be completed this Spring.
Zone 2
SCAT team conducted a detailed assessment of each segment on the
stream. The assessment was provided to the UC and Operations and included observations
and treatment recommendations on the stream. Per the Zone 2 Exit Plan, SCAT and Ohio EPA will be conducting the
inspection of the stream once Operations feels a segment of stream meets
Zone 2 criteria and, if necessary, approval for release.
Operations aggressively removed debris in the stream, especially segments 2A-02, 2B-01R and 2B-01L. SCAT described a new segment called 2B-01-SEEPAGE. This area is showing oil seeping up through the ground surface. Cardno ENTRIX to determine local geology to determine if there is a subsurface pathway.
Operations continued recovering oil and oil-debris at the underflow dam. Hull to design and construct a larger underflow dam.
Zone 3
Manual removal of residual oil and sheen and debris continued at the pond. Operations continued to clean oil debris from stream section between the pond and the underflow dam.
Hull collected surface water samples for laboratory analysis from
two locations on the pond on a daily basis. Hull also collected a
water sample from the discharge water from the underflow dam outlet.
Air sparging operations continued, as three aerators were placed and operating into the pond.
Zone 4
Daily inspections continue with no oil or sheen observed.
RECOVERY UPDATE
Total
liquid recovery was 37,850 gallons (oil and water), of which 18,693 gallons was oil.
Other Information
EPA START contractor continued assisting with assessment, documentation, sampling and monitoring activities (Field Observer).
Hull completed rotosonic drilling to install 9 groundwater monitoring wells around the site. The wells will be developed and sampled for BTEX analysis.
An air monitoring plan continued to be implemented. 11 air sampling
locations being monitored. No issues with elevated VOCs being observed.
The
Wildlife/Sensitive Species Plan for managing wildlife encountered
continued to be implemented. Cardno ENTRIX implementing the
Salamander Habit and Survey Action Plan. Tri-State Bird & Rescue on
site to assess and respond to any animals discovered which are
impacted. 57 total wildlife captured and assessed.
Environmental Unit reported additional waste being shipped off site. 6 rolloff boxes remain on site and are awaiting shipment for disposal.
Logistics signed a lease for office space at a facility approximately 5 miles from the current command post. The current command post tents will be cleaned out by Friday April 4th and will be demobilized on Saturday April 5th by Hamilton County.
RP estimated 82 total people on site.
2.1.3 Enforcement Activities, Identity of Potentially Responsible Parties (PRPs)
Sunoco Pipeline LP and Mid-Valley Pipeline are the responsible parties. A Notice of Federal Interest was issued by OSC Renninger to Sunoco Pipleine LP representatives at 0800hrs on 3/18/2014.

security badging process has been fully implemented. All personnel are
required to have a badge and as they enter the site, the badges are
scanned.

The Incident Command Post for Emergency Response activities was located in Dravo Park on 9225 East River Road.
