On 5/26/2016, the OSC mobilized to the site, at the request of the Kentucky Department for Environmental Protection (KDEP). The KDEP Environmental Response Team first discovered the spill on 5/24/2016, and filed NRC Report #1148602.

Upon arriving at the site, the OSC observed an uncontrolled oil discharge into surface water coming from an abandoned well, located within 3 feet of the creek. The OSC initiated an oil removal action, directing the EPA Region 4 Emergency and Rapid Response Services (ERRS) contractor CMC, Inc. to mobilize with personnel and equipment necessary to contain the oil spill in Duck Fork..

The location is along just off KY State Route 399 at Milepost 0.2 in Beattyville, Lee County, KY. The Duck Fork to Sturgeon Creek empties into the Kentucky River less than 6 miles downstream of the abandoned well. The Kentucky River is a navigable water of the United States.
