1.1.1 Incident Category

Oil Spill into navigable waters of the United States. Originally reported as 2000 bbls of produced water and 240 bbls of oil, the estimates were soon updated to be approximately 200 bbls of crude oil on land, and 100 bbls of crude oil spilled into the bayou. The cause of the discharge appears to be the failure of a dresser coupling on a 750 barrel capacity gun barrel tank

1.1.2 Site Description
The Site, the Huff A Unit, is an oil production facility consisting of a well, initial separation (gun barrel), storage tanks and salt water disposal well. The facility is adjacent to Highland Bayou.

1.1.2.1 Location

The incident occurred at the Dune Energy/Dune Operating Company, Huff (A) Lease. The facility, production facility, located at 6402 Haden Drive, Hitchcock, Galveston County, TX. Coordinates are 29° 21' 01" N and -095° 00' 16" W

1.1.2.2 Description of Threat
At 0745 hours on March 22, 2012, Dune Energy Inc. located on Highland Bayou in Hitchcock, TX discovered a discharge and although originally reported as 2000 bbls of produced water and 240 bbls of oil, the estimates were soon updated to be approximately 200 bbls of crude oil on land, and 100 bbls of crude oil spilled into the bayou. The cause of the discharge appears to be the failure of dresser coupling on a 750 barrel capacity gun barrel tank. Primary threat is to fish and wildlife resources in and around the bayou. Additionally, the banks of the bayou are lined with rural residences, many of which have farm animals using the bayou or drinking water.

1.1.3 Preliminary Removal Assessment/Removal Site Inspection Results

OSC Smalley and START contractors responded to the incident and met with Texas General Land Office, US Coast Guard and Dune Energy representatives. Although originally reported as 2000 bbls of produced water and 240 bbls of oil, the estimates were soon updated to be approximately 200 bbls of crude oil on land, and 100 bbls of crude oil spilled into the bayou.

Approximately, 1.5 miles of shoreline have been impacted. Texas Fish and Wildlife has visited the scene to assess the impacts. Currently no known fish or wildlife impacts have been documented.

2. Current Activities

2.1 Operations Section

2.1.1 Narrative

2.1.2 Response Actions to Date

Garner Environmental was hired as the OSRO with OMI Environmental Solutions as subcontractor. Additionally, O'BRIEN'S Response Management is providing consultation on removal activities. Containment operations commenced at 0930 and containment boom successfully deployed 1.0 NM upstream and .5 NM downstream of the spill site. The OSROs have 10 boats, approximately 40 response personnel, and 8 drum skimmers on scene, and have deployed 1400 ft of containment boom. All oil in the bayou has been contained between the FM 519 bridge, and .5 miles west of the Fairwood Rd Bridge.

Skimming operations continued all night Thursday 3/22/12 and a majority of the free phase oil was collected by pushing the oil with prop wash from the boats and water pumps, to skimmer locations to maximize skimming operations.

Skimming activities continued at the same resource level Friday 3/23/12 and Saturday 3/24/12. By Sunday 3/25/12 the free phase oil had been removed to the point that Operations shifted from skimming and vacuum trucks to sorbent boom and pads. After reviewing the progress, Federal and State Representatives agreed that recovery operations should go in to the maintenance phase. Therefore, Tuesday 3/27/12 through Monday 4/2/12, Operations consisted of a reduced crew checking and changing out sorbent pads and boom as necessary.

On Monday 4/2/12, USCG representatives attended the final site walk for EPA. State and Federal Representatives were satisfied with the removal action and approved the removal of the containment boom.

On 4/23/12, EPA Region 6 received a report on the final site and soil remedial activities from Dunes contractor, Conestoga-Rovers & Assoc., which provided a final calculated spill volume of 2,000 barrels of produced fluids, of which 220 barrels were oil. 100 barrels of oil was recovered from the bayou, via skimmers and vacuum truck.
