1.1.2 Site Description
On December 10, 2012 at 8:02 pm, Kinder Morgan reported a 13,000 gallon Sodium Hydroxide spill to the National Response Center. National Response Center Incident Report #1032879. The Kinder Morgan facility is located at 3806 Kellog Avenue, Cincinnati, OH. The facility is located along the Ohio River on the east side of Cincinnati.

US EPA, Ohio EPA, Cincinnati Fire Department, and US Coast Guard responded to the spill. Kinder Morgan hired Clean Harbors to respond to the spill.

    2.1.1 Narrative
On December 10, 2012 at 8:02 pm, Kinder Morgan reported a 13,000 gallon Sodium Hydroxide spill to the National Response Center. National Response Center Incident Report #1032879. The Kinder Morgan facility is located at 3806 Kellog Avenue, Cincinnati, OH. The facility is located along the Ohio River on the east side of Cincinnati. 


2.1.2 Response Actions to Date
On December 10, 2012 at 8:02 pm, Kinder Morgan reported a 13,000 gallon Sodium Hydroxide spill at their facility at 3806 Kellog Avenue.  Kinder Morgan also made the following notifications on December 10, 2012:

US EPA Region 5 spill hotline @ 8:07 pm
Ohio EPA spill hotline @ 8:13 pm
Cincinnati Fire Dept @ 8:21 pm
Hamilton County LEPC @ 8:49 pm

Kinder Morgan reported a Sodium Hydroxide spill involving tank #26 at the facility.  Recent maintenance work was completed on Tank #26.

On December 10, 2012 @ 8:30 pm, US EPA OSC Renninger rec'd spill notification from Phone Duty Officer Steve Faryan about the spill.  OSC Renninger completed the following response actions on December 10, 2012:

@8:35 pm contacted USCG Cincinnati Nick King and rec'd an update that USCG was responding to the spill and would update OSC Renninger upon arrival.

@8:45 pm contacted Ohio EPA OSC Farmer and rec'd an update that Ohio EPA was responding to the spill and would update OSC Renninger upon arrival.

@8:50 pm contacted Kinder Morgan Jason Dellinger and rec'd an update that the spill was contained in the secondary containment area at Tank #26.

@ 9:00 pm contacted Kinder Morgan Brandon Cox and rec'd an update that 13,000 gallons of sodium hydroxide was spilled.

@ 9:05 pm contacted Kinder Morgan Jason Dellinger and rec'd an update that the facility was removing remaining Sodium Hydroxide from tank #26 and it would take 7 hours to complete.  OSC Renninger requested Kinder Morgan to mobilize a contractor to stabilize the spill, prevent discharge to the Ohio River, and excavate contaminated soil.

@ 9:35 pm contacted Kinder Morgan Jason Dellinger and rec'd an update that the off loading of Tank #26 continued and that response contractor Clean Harbors was en route to initiate stabilization actions as requested by US EPA and Ohio EPA.

@ 10:15 pm contacted USCG Nick King.  USCG reported spill was contained to Tank #26 diked area.

@ 1130 pm contacted Ohio EPA OSC Dale Farmer and rec'd an update that Clean Harbors initiated response actions at Tank #26.  Ohio EPA investigated the outfall and requested boom placement adjacent to the Ohio River to prevent discharge.

On December 11, 2012, US EPA OSC Renninger, Ohio EPA OSC Dale Farmer, Cincinnati Fire Dept Daniel Wolf and Rick Jones and Kinder Morgan representatives reviewed the response actions and toured the Tank #26 area.  Clean Harbors activities included removing Sodium Hydroxide and water from the secondary containment area with a vac truck, excavating contaminated soil into a roll-off box, closing the overflow discharge lines, and placing sorbent boom adjacent to the overflow discharge adjacent to the Ohio River. 

Ohio EPA will monitor spill response actions thru completion.  US EPA will review Facility Response Plan (FRP) issues with Kinder Morgan.
